port\u00e9e par la coordination de la fili\u00e8re g\u00e9riatrique du sud Haute-Marne\u00a0:<\/span><\/p><p><span style=\"color: #00003f;\">\u00ab\u00a0Cr\u00e9ation et mise \u00e0 disposition d\u2019un document r\u00e9capitulant l\u2019offre de soins hospitali\u00e8re g\u00e9riatrique sur le secteur sud Haute-Marne \u00e0 destination des professionnels de sant\u00e9\u00a0\u00bb.<\/span><\/p><p><span style=\"color: #00003f;\">Ce document a pour objectif de\u00a0:<\/span><\/p><ul><li><span style=\"color: #00003f;\">Limiter les passages inadapt\u00e9s aux urgences de la population \u00e2g\u00e9e de + de 75 ans.<\/span><\/li><li><span style=\"color: #00003f;\">Orienter au plus pr\u00e8s de leur domicile la population \u00e2g\u00e9e de + de 75 ans.<\/span><\/li><li><span style=\"color: #00003f;\">Offrir \u00e0 la population \u00e2g\u00e9e de + de 75 ans du territoire une \u00e9quit\u00e9 de prise en soins.<\/span><\/li><\/ul><p><span
