    Cr\u00e9e en 2012, le service offre une capacit\u00e9 de 5 places.<\/span><\/p>

    L\u2019objectif de ce service est le maintien \u00e0 domicile des personnes \u00e2g\u00e9es et adultes (pr\u00e9sentant des pathologies neurod\u00e9g\u00e9n\u00e9ratives, scl\u00e9rose en plaques, maladie de Parkinson, AVC, d\u00e9pression de la personne \u00e2g\u00e9e) et\/ou de mettre en place des solutions de r\u00e9pits aux aidants et de relais d\u2019aide \u00e0 domicile.<\/span><\/p>

    Le territoire d\u2019intervention s\u2019\u00e9tend sur toutes les communaut\u00e9s de communes du Sud haut-marnais (Troisfor\u00eats, Chaumont, Meuse-Rognkn,Grand langres, Auberive Vingeanne et Savoir Faire).<\/span><\/p>

    Le service travail en \u00e9troite collaboration avec les partenaires m\u00e9dicosociaux de ville CM2R Dijon et la consultation m\u00e9moire du CH.<\/span><\/p>

    Missions du service\u00a0:<\/span><\/strong><\/p>

    L\u2019ESAD est charg\u00e9 de dispenser des soins de r\u00e9\u00e9ducation et de r\u00e9habilitation des formations cognitives et motrices restantes, des soins d\u2019accompagnement pour am\u00e9liorer ou pr\u00e9server l\u2019autonomie, des strat\u00e9gies de compensations. L\u2019accompagnement est personnalis\u00e9.<\/span><\/p>

    Les s\u00e9ances se font au domiciles d\u2019une dur\u00e9e de 60 \u00e0 90 minutes, une fois par semaine, sur prescription m\u00e9dicales du m\u00e9decin traitant ou d\u2019une sp\u00e9cialit\u00e9(g\u00e9riatre, neurologue)sur une dur\u00e9e de 3 mois.<\/span><\/p>
