Il ne s\u2019adresse qu\u2019aux patients \u00e0 partir de 60 ans ou aux adultes handicap\u00e9s (sur d\u00e9rogations sp\u00e9ciales) L\u2019admission est prononc\u00e9e en fonction des places disponibles et des crit\u00e8res sp\u00e9cifiques<\/span><\/p><p><span style=\"color: #00003f;\">Le fonctionnement n\u2019est pas possible sans la collaboration de la famille ou d\u2019aidants de proximit\u00e9*Le service travail en collaboration et coordination avec des partenaires m\u00e9dicaux de ville.<\/span><\/p><p><span style=\"color: #00003f;\">Les demandes peuvent venir aussi bien des m\u00e9decins hospitaliers que des m\u00e9decins lib\u00e9raux.<\/span><\/p><p><span style=\"color: #00003f;\">Le p\u00e9rim\u00e8tre d\u2019intervention des quatre v\u00e9hicules du SSIAD englobe 76 communes r\u00e9parties sur 4 cantons. Le service contribue ainsi a retarder l\u2019admission dans des \u00e9tablissements sociaux et m\u00e9dico-sociaux.<\/span><\/p><p><span style=\"color: #00003f;\">Les visites au domicile des patients se d\u00e9roulent en deux temps, selon le projet de soins personnalis\u00e9 \u00e9tabli\u00a0:<\/span><\/p><ul><li><span style=\"color: #00003f;\">Le matin pour effectuer des soins d\u2019hygi\u00e8ne, de nursing, de confort, la r\u00e9fection du lit, et l\u2019aide au petit d\u00e9jeuner\u2026 sur 4 tourn\u00e9es.<\/span><\/li><li><span style=\"color: #00003f;\">L\u2019apr\u00e8s-midi, une seule tourn\u00e9e organis\u00e9e se d\u00e9roule globalement de la m\u00eame fa\u00e7on (nursing, change).<\/span><\/li><\/ul><p><span style=\"color: #00003f;\">Rappelons qu\u2019il s\u2019agit ici d\u2019une prestation de soins financ\u00e9e 100% par l\u2019assurance-maladie, donc sans avance de frais des patients sur prescription m\u00e9dicale du m\u00e9decin traotant ou m\u00e9decin hospitalier.
